Position Overview
The Aging Life Care Manager® serves as a primary advocate for clients and their families, overseeing and coordinating care across settings. This role involves comprehensive assessments, care planning, collaboration with healthcare providers, and hands-on support during medical events, hospitalizations, and emergencies.
This position is ideal for individuals who thrive in complex situations, value autonomy with team support, and are committed to professional growth.
Key Responsibilities
- Conduct in-home, facility, and hospital visits.
- Perform comprehensive biopsychosocial assessments.
- Develop, implement, and monitor individualized care plans.
- Advocate for clients across the healthcare continuum.
- Provide support during crises, hospitalizations, and end-of-life care.
- Navigate complex family dynamics with professionalism and empathy.
- Collaborate with physicians, hospitals, facilities, and community providers.
- Maintain current knowledge of local community resources.
- Participate in regular team meetings and supervision.
- Pursue ongoing education and professional development.
- Participate in rotational on-call coverage, including holidays.
- Adhere to professional standards of ethics and scope of practice.
Qualifications
- Bachelor's degree in health or human services field, nursing, social work, or gerontology. Master's degree preferred or combination of degree and experience working with older adults will be considered.
- Experience in geriatric social work or social services, LTC/AL or memory care facility nursing, dementia care, care management, case management, home healthcare coordination, or other aging services.
- Strong assessment, documentation, and communication skills.
- Ability to work independently in the field while collaborating with a team.
- Comfortable supporting clients and families during medical and emotional crises.
- Reliable transportation and ability to travel locally.
