The Story Behind A Silver Lining

Aging is deeply personal. Your goals, values, and concerns shape everything we do; that's why our approach begins with listening.

We take time to understand your whole story. Then we coordinate care, anticipate needs before they become crises, and translate medical complexity into clear guidance. We serve as the quarterback, keeping every piece working together.

Most importantly, we stay involved. We're partners for as long as you need support.

Because even in the most difficult times, there's always a silver lining. Sometimes it's clarity. Sometimes it's peace of mind. Sometimes it's simply knowing you're not alone.

That's our commitment.

What's included:
  • 2–3 hour in-home or virtual assessment
  • Evaluation of medical, cognitive, physical, safety, and psychosocial factors
  • Comprehensive written care plan with prioritized recommendations
  • 45-minute video or phone consultation to review findings and next steps

  • Hospital advocacy and coordination with care teams
  • Support during care conferences and discharge planning
  • Medical information clarification and decision support
  • Communication and updates for involved family members

Ongoing Care Management

Long-term stability at home requires someone who sees the whole field: coordinating care, monitoring changes, and keeping everyone aligned. We serve as your proactive partner, helping prevent crises before they happen.

For families who want continued support, we also provide ongoing facility oversight after placement to help ensure care plans are followed and concerns are addressed early.

  • Assessment and recommendations to determine the appropriate level of care
  • Facility research based on quality, fit, and real-world experience
